Purpose
The purpose of this article is to guide DIR Fluoro participants in reviewing exams requiring skin reaction assessments. The Fluoro Skin Reaction Assessment form was developed to collect data on the incidence of radiation skin reaction from high-dose single procedures. This form should not be used as an alert system to identify or track patients experiencing radiation doses above the Substantial Radiation Dose Level (SRDL) of a site, only to document information on whether the identified case(s) resulted in an identifiable reaction.
How to Access
Log in to the NRDR portal and select DIR under Quality Improvements Registries. From the DIR landing page, select Fluoro Skin Reaction Assessments under Data Management.
Form Features
Fluoro exams with a Total Ka,r >5,000mGy will be listed in a table. Table columns can be found in the Fluoro Skin Reaction Assessment Table Columns and Data Collection Fields' attachment at the end of the article.
Table is filterable by
Exam Date
Dose index value (e.g., Ka,r >XXXXmGy)
only exams with Ka,r values above a user-entered value will be shown
Follow-up status (Pending, Active, Resolved, Time Lapsed)
Pending: Follow-up = No and Today - Date of Exam < 90 days
Active: Follow-up = Yes and Skin Reaction Type = 2-13
Resolved: Follow-up = Yes and Skin Reaction Type = 1 or 98
Time Lapsed: Follow-up =No and Today - Date of Exam > 90 days
To complete a fluoro skin reaction assessment, please select the ellipsis (...) of the exam row under the Actions column, click Edit and a drop down of data collection fields will appear.

Fluoro Skin Reaction Assessment Reminders
Facility Administrators and Service Users receive emails on the second Monday of every month if there is at least one active DIR Fluoro facility with "Active" Fluoro Skin Reaction Assessment forms.
