Accepted Documentation
Upload official documentation that clearly displays the Tax Identification Number (TIN).
- Tax documents
- Submitted Medicare claims
- Other official documentation showing the TIN
Important: Do not upload documents containing banking account numbers, patient identifiers, or other unnecessary personal information.

Select TIN(s)
↓
Browse for File
↓
Upload Document
↓
Confirmation Appears in Table
- Open TIN Documentation.
- Click Upload TIN Documentation.
- Locate the appropriate TIN using the available filters if needed.
- Select one or more TINs associated with the document.
- Click Browse and select your file.
- Click Upload.
- Verify the document appears in the documentation table.

Required: At least one TIN must be selected before uploading a document.
Update or Delete Documentation
- Locate the document in the table.
- Select the appropriate action in the Action column.
- To update associations, select or remove TINs as needed.
- Click Save to apply your changes.
- Use the delete option if documentation should be removed.
