Add the facility details, administrator information, and registry selections required to participate in NRDR.
Quick Overview
- Create a Facility Information Form for each physical location that will submit data to NRDR.
- Enter facility details and designate a Facility Administrator.
- Select the registry or registries the facility will participate in.
- A unique Facility ID is automatically created when the form is completed.
Before You Begin
The Facility Information Form becomes available after you create a corporate account.
To add a facility:
- Select Add New Facility.
- Complete the required facility information.
- Enter Facility Administrator details.
- Select participating registries.
- Click Next to continue.
Tip: Fields marked with an asterisk (*) are required.
Step 1: Enter Facility Information
Provide basic information about the facility location that will submit data to NRDR.

Additional Notes
- Facility NPI must contain exactly 10 digits if provided.
- NMD-Certified Vendor information is required only for NMD participants.

Step 2: Assign a Facility Administrator
Each facility must have one designated Facility Administrator.
- May be the same individual as the Corporate Account Administrator.
- Serves as the primary facility contact.
- Receives NRDR-related communications for the facility.
- If a different person is assigned, the system sends an invitation to create an NRDR account.

Step 3: Select Registries
Choose the registry or registries to which the facility will submit data.
