A fully executed Participation Agreement and Business Associate Agreement (BAA) must be in place for a corporate account before any facility under that account may submit data to a registry. The Participation Agreement details the obligations of the NRDR and the obligations of the corporate account holder with respect to the NRDR, including the safeguarding of Protected Health Information, as mandated by the Health Insurance Portability and Accountability Act of 1996 (HIPAA).
The Participation Agreement and BAA must be signed and returned to the NRDR Administrator before your facility can submit data to a registry.
Participation Agreement
The Participation Agreement is displayed during the application process and may be signed online. Review the entire Participation Agreement carefully and, when finished, click the box next to the statement beginning with The undersigned Participant-signatory acknowledges… at the bottom of the page.
Use the Print button to print a copy of the agreement for your records, then click Next to proceed to the Business Associate Agreement.
Note: Your Corporate Account Administrator can obtain a copy of the Participation Agreement at any time from the My Applications page by selecting View Registration Report for the initial corporate account application.
Business Associate Agreement (BAA)
The BAA must be downloaded, signed, and sent to the ACR to complete your application process. If you already have a BAA on file that was signed after January 1, 2018, and was executed within the past three years, you may add any new facility IDs and resubmit that document. Otherwise, download the BAA via the link provided in the application, complete the form, and submit it via e-mail to nrdr@acr.org, by fax to 703-295-6776, or by mail to
