Data for MIPS measures are submitted to the MIPS Portal via web-based data upload using an Excel or text data file. Data files must meet the specifications described in the MIPS Data Upload File Specification documentation. The specification documents describe the filename conventions, data element names, and other file requirements. Be sure to download and review the specifications before creating your first data file, particularly if you plan to create files without using the MIPS Portal templates.
To help you create your data file, download the MIPS Measures Excel Template (attached at the bottom of the article) or a MIPS Measures Text File Template from the Upload Data page in the MIPS Portal.
Note: Text files are the preferred file format for uploads. If you prefer to work with Excel, you can create your data file as a spreadsheet and convert it to a .TXT file before uploading to the portal.

The templates allow you to enter or copy your data into an Excel spreadsheet or a pipe-delimited text file for uploading to the portal. The template contains the correct field names for each column. The file also creates a unique filename that meets the file specifications.
The MIPS Measure Code List all MIPS measures and their relevant numerator and denominator codes. Use the information here to populate the upload template with measure data.
Data for ACR QDCR measures are submitted through the relevant process for each registry; methods vary by registry. Consult the data submission articles for CTC, DIR, IR, GRID, LCSR, and NMD for more information. You do not need to submit data to all the NRDR registries; only submit to registries that support measures relevant to your practice for MIPS participation.
