NRDR Uploader App: Patient Upload

The Patient Upload function allows you to add or change multiple patient records at one time, rather than doing so one by one on the web page.


Changing Patient Data

Start by downloading an Excel file of your existing active patients. Ensure that you are working with the correct Facility ID:

The file has a header for each column showing the permitted values for that column. The text color of the header indicates whether the column is required or not:

  • Gray – populated by the system and should not be changed.

  • Black – optional

  • Blue – conditionally required. For example, Patient Other ID is required if no other patient identifier is reported. Otherwise, it is optional.

  • Red – required

Change each record as needed. Ensure that the changes conform to the requirements shown in the column headers. It is not necessary to delete unchanged records from the file, but doing so may reduce processing time.

 

Drag and drop the file to area indicated, or upload the file using the browse function:

 

Your file will appear in the list of uploaded files, along with information about the upload status, the percentage of records processed, number of records in the file, and the number of records added, updated and rejected:

 

If there are errors in the file, and there are less than 50,000 errors, a Download Log File button will appear in the Action column. You can download the log file to view the errors. You can correct the errors directly in the log file and reload it.


If there are 50,000 errors or more, a Generate Log File button will appear in the Action column. By clicking this button, you can request that a log file be generated. You will receive an email notification when the log is ready for downloading. At that time, the button will change to Download Log File.


Note:

  • It is not possible to delete a patient or change a patient’s status using the upload function. Use the Patients tab to perform those functions.

  • Patients from different facilities cannot be included in the same file.

 


You can use the filter to search for file uploads by status and submission date:


 

Adding patients

New patients can be added by entering their information into blank rows in the file of your existing patients. If you leave the Facility ID blank, it will default to the Facility ID with which you are logged in. You must leave the NRDR Patient ID blank. All other entries must conform to the requirements described in the column headers.


Note: It is not necessary to add patients in this way. Whenever you upload records for a new patient to registries in the NRDR, the patient’s information will automatically be added to your patient list.
