A signed Business Associate Agreement (BAA) is required for each corporate account. If you do not currently have a BAA on file with the ACR, you may download a copy here. Fill in your Corporate Account ID number, Facility ID numbers, sign the form, and submit it using one of the following options

If you already have a BAA on file with the ACR, either for the NRDR portal or for another program, such as Accreditation, you may send and use that agreement for the NRDR portal or to add new facilities.
Note: You may use an existing BAA provided the document was signed on or after January 1, 2018, and was executed less than three years ago.
The ACR cannot accept revisions to the BAA and will not review third party BAAs.
