The purpose of this article is to outline functions to manage patient information in the NRDR Uploader app.
Add New Patient
To add a new patient, click Add New Patient in the Patients tab and click Save once the patient information has been entered. The below fields will populate for data entry:
Last Name (optional)
First Name (optional)
Middle Name (optional)
SSN (show as xxx-xx-xxxx)
if duplicate SSN is found, a notification will appear with the NRDR patient ID of the existing patient.
Required if no other patient ID is entered (Old MBID, New MBID, Other ID). Optional otherwise.
Old MBID
Prior to April 2018.
Required if no other patient ID is entered (SSN, New MBID, Other ID). Optional otherwise.
New MBID (show as xxxx-xxx-xxxx)
After April 2018.
Required if no other patient ID is entered (Old MBID, SSN, Other ID). Optional otherwise.
Other ID
Required if no other patient ID is entered (Old MBID, New MBID, SSN). Optional otherwise.
Date of Birth (optional)
Patient Sex (required)
Race (optional)
Hispanic Origin (optional)
Date of Death (optional)
How cause of death was determined (optional).
Cause of death (required if date of death determined).
Health Insurance (optional)
Educational Level (optional)
Edit Patient
To edit a patient in the Uploader app, click Edit in the Action column for the corresponding patient. The patient must have an active status to edit. Please refer to the list of fields in the Add Patient section of this article for fields that are editable. Click Save once desired fields are edited.
Delete Patient
To delete a patient, click Edit in the Action column for the corresponding patient then click Delete. You can also delete a patient by clicking the Delete button in the Action column in the populated table of patients. A prompt will appear requiring a confirmation to delete the corresponding patient.
Note: Patients cannot be deleted if they have existing exams in NRDR.
Cancel Patient
To cancel a patient, click Cancel Patient in the Action column for the corresponding patient in the Patients table.
Note: It is recommended that a patient only be canceled if no other exams will be submitted for the patient. Submitting exams after a patient has been canceled will open a new active record for the patient.
Activate Cancelled Patient
To make a patient active again, click Make Active in the Action column for the corresponding patient in the Patients table.
Note: The system will validate that the existing ID for this patient is still unique to the specific patient. If no duplicates are found, the system will activate the cancelled patient sucessfully. If there is a duplicate patient found, an error message will show and the patient will not be changed to active.
Merge Patients
To merge patients via the web form, search for the patient(s) by using the filter function (first name, last name, SSN, etc). Select the patients you wish to merge by clicking the checkbox next to each patient, or click Select All if you wish to merge 100 patients or less. Select Merge Patients and Confirm Merge Patients to complete the merge.
Note: No more than 100 patients can be merged at once. If more than 100 patients are selected manually, or the Select All button selects more than 100 patients, an error will display and the merge patients function will not work. If you wish to merge more than 100 patients, please use the merge patient data file upload function to merge patients. There must be at least one active patient in the selected list to merge patients. If there are only cancelled patients selected, the merge patients function will not work.
Potential Patient Duplication Report
The Potential Patient Duplication report allows user to identify existing patient records that may refer to the same patient. To access this report in the Patients tab, select Duplication Report button. Follow the steps in the Merge Patients section if you wish to merge your duplicate patients.
View History of Patient Changes
To view changes made to the patient profile (additions, edits, deletions, modifications), select History in the Action column or View History under the Patients table.
