Please note the following when entering data:
Dates must be entered in mm/dd/yyyy format.
Data elements defined as alphanumeric must be limited to the characters A - Z, a - z, 0 - 9, or _ (underscore) only.
Entries must not include leading or trailing blanks.
For information regarding the format used for entries on specific forms, refer to the registry-specific documentation available in the Submitting Data folder for that registry – e.g. Submitting Data to CTC. You will find articles with tables describing the format of the entries found on the form.
Clicking the
button that appears next to most entries will display format information in many cases.First and last name fields must be 45 characters or fewer. At least two characters must be from the characters A - Z, a - z, or ‘ (single quote). Additional characters may include - (hyphen).
