New NRDR Participants

All new NRDR participants must submit a fully executed Participation Agreement, including a Business Associate Agreement (BAA). The Participation Agreement details the obligations of the NRDR and the obligations of your facility with respect to the NRDR, including the safeguarding of Protected Health Information, as mandated by the Health Insurance Portability and Accountability Act of 1996 (HIPAA).

The Participation Agreement and BAA document must be signed and returned to the NRDR Administrator before your facility can submit data to a registry.

Download and complete the Participation Agreement and BAA if this is your first time using any registry.

Note: You can use one Participation Agreement and BAA document even if you are planning to sign up for for multiple registries at one time. Do not submit the Addendum document unless you have already started using the NRDR and need to alter your participation to cover additional registries. 


If you are already contributing to one or more NRDR registries and plan to use another registry, download, complete and submit an addendum to your Participation Agreement to document to which new registry/registries you will submit data, and to confirm your obligation regarding the data to be submitted.

Note: You only need to submit an addendum if you are already using another registry and want to amend your participation agreement to use another registry.
